2. What information we collect

Health records are retained for the periods required by law (generally at least 7 years from the last entry, or until age 25 for minors).
7. Accessing and correcting your information
You have the right to request access to the personal information we hold about you, and to ask us to correct it if it is inaccurate, out of date or incomplete. Requests should be made to the practice in writing. We may charge a reasonable fee for retrieving and copying records.
