Terms & Conditions of Treatment

These terms apply to dental treatment provided by Westside Dental at Westside Medical Centre, 25 Loudon Road, Port Augusta West SA 5700. By attending an appointment you agree to these terms. The responsible entity and ABN will be inserted here once provided.

1. Care and standards

We provide dental care in accordance with the Dental Board of Australia’s codes and guidelines and the Australian Health Practitioner Regulation Agency (AHPRA) requirements. Nothing in these terms limits any rights you have under the Australian Consumer Law or other rights that cannot lawfully be excluded.

2. Informed consent

Before treatment we will discuss the proposed care, the likely benefits, material risks, reasonable alternatives and the expected cost. You may ask questions at any time and may withdraw consent before or during treatment. Some treatments require written consent.

3. Medical history

It is your responsibility to provide accurate and complete medical and dental history, including medications, allergies and relevant conditions, and to update us if anything changes. This is essential for your safety.

4. Fees and quotes

  • We are a private practice and our fees reflect the time, materials and clinical expertise involved in your care.
  • A written treatment plan and estimate will be provided for treatment beyond a routine examination and clean. Estimates are a guide; fees may change if the clinical situation changes, and we will discuss this with you before proceeding.
  • Health fund rebates vary by fund and policy. We are not responsible for the level of rebate your fund pays.

5. Payment

Payment is required at the time of treatment unless other arrangements have been agreed in writing in advance. We accept eligible Medicare (Child Dental Benefits Schedule), DVA and private health fund claims where applicable. A deposit may be requested for laboratory-based work such as dentures.

6. Cancellations and missed appointments

Please give us at least 24 hours’ notice if you need to change or cancel an appointment. See our Cancellation & Missed Appointment Policy for full details.

7. Children and dependants

For patients under 18, a parent or legal guardian must provide consent and, where practical, attend the appointment. The accompanying adult is responsible for payment.

8. Your responsibilities

  • Arrive on time and provide accurate information
  • Follow the home-care and post-treatment instructions provided
  • Treat our staff and other patients with respect — we may decline to treat anyone behaving abusively

9. Concerns and feedback

If you are not satisfied with any aspect of your care, please speak with us first so we can address it. You may also contact AHPRA or the Health and Community Services Complaints Commissioner (HCSCC) of South Australia.

Information you provide is handled in accordance with our Privacy Policy.
